Agency Forms
The Washington State Patrol is responsive to requests from our citizens
for agency forms. To obtain a copy of a WSP form, please use the
following procedures:
Motor Vehicle Collision Report Form - If you wish to file a report for a collision, please go to the Collision Reports page and
follow the form instructions to submit to the Washington State Patrol.
If you were involved in a collision and need a copy of the collision report, follow the instructions on the Collision
Reports page.
For any other collision reporting form, please contact the WSP Collision Records Section at (360) 570-2355.
Uniform Washington State Authorization to Tow/Impound and Inventory Record - Please order through the State Printer's Web site at http://www.prt.wa.gov/. Once
there, click on "Online Ordering," then "General Store."
Criminal History Background Checks - For criminal history records and related forms, please see the Washington Access To Criminal History (WATCH) on-line or contact watch.help@wsp.wa.gov.
All Other WSP Forms - If you don't see the form you are looking for in the list of links below, please contact the agency Documents Manager.
