Data Collection · NFIRS & Fire Fatality Reporting

Due to budget cuts, effective July 1, 2011, the State Fire Marshal’s Office will no longer provide support to the National Fire Incident Reporting System (NFIRS). Fire agencies may continue to submit their NFIRS data directly to the United States Fire Administration Data Center. To submit your data directly, visit the U.S. Fire Administration's webpage at www.usfa.dhs.gov/fireservice/nfirs.

Agencies may contact the Washington Fire Chiefs Directly for assistance at: wfc@washingtonfirechiefs.org.

Annual Reports & Fire Fatality Statistics:

Fire Fatality Reporting

RCW 43.44.060 requires any death resulting from fire to be reported to the Office of the State Fire Marshal within two business days. A short form has been developed to assist agencies in providing this important statistical information.

You may submit the information by phone or in writing on the Fire Fatality Report Form provided below. Address your report to the attention of Deputy State Fire Marshal Lysandra Davis.

Note: Some forms have been provided in Microsoft Word format to allow submitting by email. If you do not have Microsoft Word, you can download the current Word Viewer here.
